4.To know the name, identity and professional status of all care providers involved.
5.To know progressive information on the diagnosis, treatment and any prognosis.
6.To have access to clinical records.
7.To insist to records treat an all communication confidential. 8.To learn care plan, adverse consequences, estimated cost, expected results and alternates.
9.To learn immunization needs.
10.To learn about prevention of infection.
11.To know the tariff of different care plans.
12.To request to perform the spiritual or personal beliefs related desires.
13.To participate in discharge planning process.
14.To raise complaints if any on care process and to get resolved through PRO.
15.To demand for continuity of treatment.
16.To demand for details/clarification on bills.

Responsibilities:-

1.To provide accurate and complete information on medical history and health problems.
2.To participate in the care plan preparation by care providers.
3.To seek clarifications on doubts if any.
4.To abide by the advices of care providers and to cooperate for treatment process.
5.To understand the consequences when denying the treatments
6.To be considerate for providing care to other patients.
7.To abide by hospital rules and regulations like smoking etc.
8.To cooperate for prompt bills settlements as per rules of the hospital. 9.To cooperate for keeping the premises and other utilities clean
10.To cooperate the hospital staff to perform their duties.
11.To follow instructions notified from time to time by hospital management.
